फॉर्म कैसे सेव करें?

How to download form from TJA blog?
Just take your mouse arrow on the form... right click on it and choose save the picture option, choose drive (C:or DESKTOP) before you save it.
This will save file on your drive , then just open the file, print it.... fill it and send it to the persons name mention below.
